Please be sure to include the defects the child was born with, and also include details about the pregnancy and delivery.

  Was pre-eclampsia present?  
  Was the child delivered early or with low birth weight? 
  Did placenta abruptia occur? 
  What was the mother's age, height and weight at the time of delivery? 
  Does the mother have a history of snoring or pauses in breathing during sleep?
  Is there a family history of sleep disordered breathing (snoring and sleep apnea)? 
  Is there a family history of disease or birth defects among other family members?
